Life

Life appointment requests are processed through the Duncan Advisor Resources SureLC contracting system. If you already have an existing DAR SureLC profile, click the Request Appointment link to contact our contracting department.

If you have not set up your profile in our SureLC environment, please click on the Life Contracting Profile button and follow the prompts to set up your account. You will need to select New User, even if you have a SureLC profile set up with another agency. Your information will transfer in from your existing profile once you create your DAR account.

Once you have setup your profile, click the Request Appointment link to contact our contracting department.

Disability Income

Disability Income appointment requests are processed through our partner, the Cohen Agency.

To request an appointment, please initiate the process with your Income Protection Specialist by clicking on the Request a DI Appointment button to the right.

Long Term Care

(Stand Alone)

Long Term Care* appointment requests are processed through our partner, the Cohen Agency.

To request an appointment, please initiate the process with your Income Protection Specialist by clicking on the Request a LTC Appointment button to the right.

*Stand-alone policies only. Hybrid LTC products should be submitted through the Duncan Advisor Resource Life Brokerage.

Annuities

Annuity appointment requests are processed through the Simplicity Financial Marketing SureLC system.

If you are not already working with your dedicated Business Coach, Brian Rieman, please contact him at brianrieman@simplicitymarketing.com to discuss carrier and product requirements prior to requesting an appointment.

Once you have determined your contracting needs, please click on the Request an Annuity

Appointment button to access the Simplicity SureLC environment. This is different than the DAR SureLC system and you will be prompted to login with your Simplicity specific credentials.

Medicare

Medicare appointment requests are handled through your JSA Marketing Representative, Chris Jones.
